The Childhood Immunisation & Health Inequality Lead role isan exciting opportunity that combines responsibilities from two key areas:increasing childhood immunisation coverage and addressing health inequalitieswithin the our resident population. The role focuses on improving healthoutcomes for children under 5 through immunisation efforts while also workingon broader health inequality projects that target disadvantaged populationswithin the Springfield Park PCN areas.
Main duties of the job
This position requires a proactive approach to both clinical servicedelivery and community engagement, ensuring that underserved groups receiveappropriate healthcare interventions. The role involves close collaborationwith PCN staff, local healthcare providers, voluntary organizations, andcommunity groups.
About us
Groups of GP Practices in City and Hackney have come together toform 8 Primary Care Networks (PCNS) each covering a total population of between30,000-56,000. The purpose of these PCNsis to enable GP practices to work together in a collaborative way to developand deliver network-based services that respond to the needs of the localpopulation.
Job responsibilities
Childhood Immunisation Responsibilities:
- ImmunisationDelivery: Provide childhood immunisations forunder-5s, including Flu, HPV, MEN ACWY, and Td/IPV, MMR in clinicalsettings and through outreach and domiciliary visits.
- Coordination: Work with practices to organize immunisationclinics, manage patient invitations, and coordinate care for specificpatient cohorts, especially those hard to engage.
- DataCollection & Analysis:Gather and analyse data on immunisation rates, and contribute todeveloping strategies to improve uptake.
- ClinicalSupport: Adhere to infection control procedures,manage clinical incidents, and ensure proper communication andcoordination with the broader healthcare team.
- Education& Training:Provide training to practice staff on immunisation protocols and supportongoing professional development.
Health Inequality Responsibilities:
- CommunityEngagement: Identify and engage with communitiesexperiencing health inequalities, particularly focusing on ethnicminorities, economically deprived groups, and those with chronic healthconditions.
- Data-DrivenInterventions: Utiliseavailable data to identify health disparities within the PCN and designtargeted interventions such as Des- CORE20PLUS5
- ProgramImplementation: Leadthe implementation of health inequality initiatives such as improving CVDmanagement, increasing cancer screening uptake, and enhancing access tomental health services.
- Collaboration: Work with local system partners to addressbarriers to care, develop community assets, and create sustainable healthimprovement strategies.
- Evaluation& Reporting:Establish processes to evaluate the impact of health inequalityinterventions, report on progress, and adjust strategies as needed.
- Tobe responsible for ensuring community engagement work takes place ensuringaims and objectives are met.
- Systemlevel: to represent the general practiceneighbourhood and community partners in the ICS inequalities improvementagenda.
- Placelevel: supporting the integration of primary care,secondary and community services, in collaboration with peerorganisational HI Leads, whilst driving the change to help mitigateinequality. Engaging with other place- based partnerships.
- Neighbourhood/ Community Level: tobuild collaborative, sustainable and positive relationships with externalpartners to develop a shared vision and understanding of the issues andchallenges and how to work together to meet the needs of the communityensuring that it is from a population perspective.
KeyRelationships
- Internal: PCN staff,doctors, trainees, and administrative personnel.
- External: Local health authorities, voluntary sectororganisations, community groups, patients, and other key stakeholdersinvolved in addressing health inequalities.
Person Specification
Qualifications
- Registered Nurse (RN) with NMC registration (child branch preferred).
- Experience in community health, especially working with children and disadvantaged populations.
Experience
- Demonstrated experience in administering immunisations and working independently within community settings.
- Operational leadership experience would be desirable
- Strong understanding of health inequalities and experience in designing and implementing public health interventions.
